Contestant must get on a pole directly out, under the nose of a helicopter. Players will bob in cheese and then eat grasshoppers. Contestants are hung upside down into a Plexiglas tank filled with Eels. Contestants must drive up with only two wheels.

Contestants hold onto monkey bars, suspended under a helicopter. In a Plexiglas coffin and covered with leeches. Contestants search for rubber boots at the bottom of a tank of cut up fish parts. Contestants must run their motorcycles off of a cliff.

Semi Circle Flag Snag,Feeding Frenzy With Piranhas,Roulette Spider Eating,

Contestants will start on the roof of The Mandalay Bay. A tank is filled with Piranha and pig kidneys, which will need to be picked up off using their mouth. The contestants are allowed to eat up to six cave spiders.

Taxi to Taxi Jump, Roach Coffin, Crain Money Grab, BlackJack Bet,

Taxi cabs each hang from their own cranes. We place the player inside a sealed Plexiglas coffin filled with thousands of roaches. The contestants dangle by their ankles from a crane, suspended over a swimming pool.

Heli Crawl and Flag Grab,Maggot Madness,Head on Pipe Ramp,

Contestant seated in the helicopter will "go" they crawl out and under the helicopter. Players transfer cow hooves out of a tank of Cod Liver Oil and into a tank of Maggots. Contestant must drive their car at approx. 25mph towards another car.

Heli Water Drag,Cow Eye Juice,Truck-To-Truck Transfer,

Contestants will start this game floating in the water. Contestants compress a cows eye with their mouth, squeezing the juice out of the eye into a glass. Contestant will stand on Euclid dump trucks as they travel down the road.

Snake Tube,Goblets of Fire,Eat Stink Beetles,Bobbing In Cow Eyeballs,

Players are placed into a large plexiglas coffin with several thousand snakes. Eight glass torch-goblets are placed on stone pillars of varying heights. Contestants will bob in a tank filled with cow eyes to recover pig hearts.

Submerged Bus,Dirty Kitchen,Brain Surgery,Falling Beam Walk,

Contestant must dive into the water and swim down to a city bus. On a table, there are dishes of roaches. Players are given fifty cow brains with a FF chip hidden inside. Contestants must make the way across a maze of beams.

Water Sack,Beetle, Roach, Worm Transfer,

The contestant is placed in a specially designed clear bag. A tank is filled with millipedes, roaches and worms. Contestant is standing on one of four 8-foot tall 16-foot long Plexiglas walls suspended high above the ground.

Truck to Helicopter, Table Clearing, Under the Semi,

The contestant will be riding in the back of a truck as it drives down a pier. Players will have to clear the tables to uncover a three-digit combination written on each table's glass top. Contestant must drive their car under a semi truck.

Boat Raft Leap Frog, Family Tunnel, Worm Head, Suspended Water Tube,

There will be a boat that is towing three rafts. A Plexiglas bottom covered in pig parts. The child stands under a large metal shower-head and pulls a chain. Suspended above the water. The kid must climb through the tubes and traverse.

Boat to Boat Transfer, Worm Wine, Matrix Truck Straddle,

Contestants leap back and forth from the boats to collect as many flags as possible. A contestant stands in a vat used to stomp grapes for wine. They walk the length of the truck, removing flags along the way.

Cable Surfing,Gift Exchange,Lake O'Fire,

The contestant must slide down two cables while standing on a small platform. One at a time, contestants pick gifts from under a Christmas tree. Contestants must swim underneath a 20 foot by 40-foot Plexiglas grid that is lit on fire.

Dual Helicopter Box Drop,Bee Rescue,Submerged Car Rescue,

Contestants will be placed on a platform floating in the water, where they will grab a skid of the helicopter. Player is chained between two poles and covered with bees. Contestants will be seated in the front of a car suspended over the water.

Dune Buggy Drag, Bury Spouses Head, Heli Drop and Raft Pull,

The contestant will be attached to a dune buggy by his ankles. Contestant will have to bury their spouse's head in worms using only their mouth. One person from each team will be hanging from the helicopter's skids.

Couples Chain Submerge, Sewer Transfer,

Contestant will be chained to either side of a platform with the contestant having one key to free them from chains. Player dunks their head into a tank and retrieves a cows heart with their mouth.

Spouse Spin,Fear Factor Pie,

In this couples stunt, partners will hang by their ankles, they will each hold onto one end of a rope and be lifted over the water. Couples will use their mouths to root around in pies made from wax worms, pigs stomach, fish sauce and duck tongue.

Gross Dunk Tank,Heli Spouse Slalom,

Player A is placed on a dunk tank. In the tank is a mixture of cow parts floating in water. One member of the team will be suspended by their ankles underneath a helicopter.

Heli Spouse Rescue,Dumpster Dive,

In this stunt, the female contestant will be placed in a cage that is hung from a helicopter and is approx. 20 feet in the air. A dumpster filled with disgusting garbage is placed at one end of an alley.

Lean and Grab, Bobbing in Rats, Hog Tied Mate,

The contestant will be standing on a beam that is suspended high in the air. The player is placed in a coffin with 100 chicken feet, then covered with rats. Couples stunt where one of the team members is hog-tied by four ropes and suspended.

Back Seat Driver,Save Your Spouse In Spiders,Heli Boat Jump & Plunge,

Two contestants are seated in the front of a car. The driver is blind folded and the passenger must navigate for them. Player is strapped down in a coffin and then covered with 500 Tarantulas.

Heli Crawl and Rope Slide, Suspended Floor Drop, Intestine Chew, Milk & Drink, Mini Motorcycle Plank,

The contestant exits the helicopter and releases a flag. They stand on platforms suspended in the air. The contestant chews through the cow intestine. Contestants ride a mini motorcycle across a beam that hangs from a crane.

The production team will place the contestant inside a clear plexiglass tube and lift it to just above the water. They will then dump snakes and lizards into the tube. The contestant will stand on a plexiglass incline pitched atop a roof high in the air.

Tumbler Transfer, Grab and Grind, Tanker Truck Flag Build,

Contestants stand at either end of two large cylinders suspended over water. Each player grabs a mouthful of insects and places them into a meat grinder using only their mouth. Inside the tanker truck, water fills the space.

Twirling Misery,Eat Live Snails,Car Jump Out of Back of Semi,

Contestants are placed in a box with two plexi-glass walls that will be lifted off the ground. Contestants will be served live snails. Contestants are seated in a car that is in the back of a semi truck.

Extreme Building Plunge,Bug Windshield,Rotating Beam To Platform,

Contestants will be dangling from a rope off the roof. Contestants will have to clear a bug splattered car windshield using only their mouth and face. The contestant will be standing on a beam suspended from a crane.

Tunnel Flag Snag, Potato Bug Bazooka, Tractor Car Roll,

Contestants stand atop a speeding truck, trying to grab as many flags as they can. They shoot potatoes at a target using a shoulder-mounted air cannon. The contestant starts the car while tractors drive towards them.

Family Beam Walk, Family of Roaches, Kid-a-Pult,

The parent and the child will have to cross a beam above the water, releasing flags along the way. Cockroaches are being poured into a box while the child sits in an open box. In this family stunt, the child will start by standing on the dock.
